INSTRUCTIONS: All forms must be submitted by May 1. 

The NFTY in Israel application process requires that all of the forms on this page are completed, signed (when applicable) and submitted, except where designated as optional. Most forms are filled out and submitted in your web browser, no need to print and fill out by hand (except where designated)! Forms are:

• URJ Camp and Israel Programs Medical Form - Please print, fill out by hand, and return to us via fax, email, or US mail
• Rabbinic Recommendation 
• Parents' Form - Includes Emergency Contact Information, Visitation Release, Release Letter and Authorization, Brit Kehillah Code of Conduct, and Safety and Security Release 
• Participant's Form - Includes Pre-Summer Survey and Brit Kehillah Code of Conduct 
• Passport Form
• Parents' Vacation Information
• Visitation Information - Authorization for friends and family to visit the participant during the summer, you may fill this out multiple times if you have more than three names to list or you have new names to add later on

Please note:

• All forms must be submitted no later than May 1, 2012.
• We do understand that many families have their teen's annual physical scheduled after May 1. Please submit your Medical Form as soon as possible after your teen's physical. There is no need to contact us if you will submit the medical form after May 1. 
• After completing each form, you will receive a confirmation email including the text of the form and your responses (except where confidential, to protect your digital privacy).
• Please read the instructions for each form carefully for special notes/requirements.
